QUALIFICATIONS: - Health - Ability to meet performance requirements
- Education, Experience and Skills:
- Graduate from an approved school of nursing, College or University
- Must have a current, unencumbered license to practice nursing as a Registered Nurse/Licensed Practical Nurse in Florida
- Participates in continuing education so that he/she has knowledge of current trends and developments in nursing and maintains current license
- Minimum of two years of RAI and Care Plan experience, preferably in a Skilled Nursing Facility (SNF)
- Must have knowledge of MDS 3.0 including Assessments, CAA's and Care Plans
- Conduct each Assessment and Care Plan with the appropriate participation of health professionals
- Ability to organize work procedures, assume responsibility, and tactfully interact with other disciplines
- Must be able to communicate well with any member of the Interdisciplinary Team
- Collaborate with the IDT to provide guidance on appropriate and precise charting for the medical record
PERFORMANCE REQUIREMENTS: - Must be able to sit, stand, bend, twist, lift and move intermittently throughout the day
- Must be able to lift, push, pull and otherwise move a minimum of 50 pounds
- Able to see and hear adequately in order to respond and interact with other staff and families
- Able to speak in clear, concise voice in order to communicate with patients, families, and other staff and disciplines
- Relate to and work with the elderly, disabled, emotionally upset and hostile Residents
- From time to time, you may be required to assume the duties of a floor nurse
SPECIFIC DUTIES AND RESPONSIBILITIES: - Assign responsibility for completing sections of the MDS to staff who have clinical knowledge about the patient
- Assure that non-licensed staff who provide direct care to the patients are allowed to contribute information for the assessment process
- Conducts, completes or coordinates other disciplines in completion of the RAI and certifies the completion of the Assessment
- Sign and certify the CAA Summary Form
- Attend care planning and enter the care plan decision information
- Assist with month-end duties for closeout
- Perform other assignments as assigned
