As a Licensed Practical Nurse (LPN), you will provide direct patient care to residents in a skilled nursing setting. You will work under the supervision of a Registered Nurse (RN) and collaborate with healthcare professionals to ensure the delivery of high-quality care and the well-being of our residents.
Key Responsibilities:
- Patient Care: Deliver nursing care to residents, including administering medications, performing treatments, and monitoring vital signs. Assist with activities of daily living as needed.
- Assessment and Documentation: Observe and document residents’ health status, including changes in condition, and communicate findings to the RN or physician. Maintain accurate and up-to-date medical records.
- Care Planning: Assist in the development and implementation of individualized care plans based on residents' needs and preferences. Ensure care plans are followed and updated as necessary.
- Medication Management: Administer prescribed medications, including oral, topical, and injections, and monitor for any adverse reactions or side effects.
- Communication: Provide clear and compassionate communication with residents, families, and the healthcare team. Address any concerns or questions regarding care.
- Emergency Response: Respond promptly to medical emergencies, providing appropriate care and support until higher-level medical personnel arrive.
- Collaboration: Work closely with RNs, physicians, therapists, and other healthcare professionals to coordinate care and address residents' needs.
- Patient Education: Educate residents and their families on health conditions, treatment plans, and self-care techniques.
- Compliance: Adhere to facility policies, state and federal regulations, and nursing best practices to ensure high standards of care.
- Team Support:*Assist with training and mentoring of nursing assistants and other support staff. Participate in facility meetings and quality improvement initiatives.
Qualifications:
- Education: Completion of an accredited Practical Nursing program.
- Licensure: Current and valid LPN license in Nevada.
- Experience: Previous experience in a skilled nursing or long-term care setting is preferred, but not required.
- Skills: Proficiency in basic nursing skills, including medication administration and patient assessment. Strong communication and interpersonal skills.
- Certifications: BLS (Basic Life Support) certification required; additional certifications may be preferred based on facility needs.
- Personal Traits: Compassionate, detail-oriented, and able to work effectively both independently and as part of a team.
Physical Requirements:
- Ability to lift, move, and assist residents as needed.
- Prolonged periods of standing, walking, and bending.
- Ability to handle physical and emotional stress related to patient care.
