OhioGuidestone is currently looking for qualified individuals to fill a PRN (as needed) HYBRID, Diagnostic Intake Assessor position in Perrysburg, OH. This position will be responsible for completing diagnostic assessments on a scheduled or walk-in basis.This position is not responsible for managing an ongoing therapy caseload and therefore can provide flexible schedule options including an option of only evenings/weekends - Great Second Income!! Ideal candidates must possess, at minimum, an LSW with Bachelor's degree. EDUCATION/EXPERIENCE/LICENSE/CERTIFICATION Required: - LSW with Bachelor's degree in Social Work
Preferred: - LPC/LPCC or LSW/LISW with Master's degree
- Experience in completing diagnostic assessments
ESSENTIAL DUTIES AND RESPONSIBILITIES - Provide diagnostic assessments on a scheduled or walk-in basis to new clients.
- Complete treatment recommendations and treatment planning after initial diagnostic assessment.
- Integrate Trauma Informed Care practices while working with persons served.
- Communicate with relevant internal and external care providers to coordinate care.
- Communicate with referral sources to gather necessary collateral information and verify client's completion of requirements made by referral source.
- Maintain complete and current clinical records regarding all persons served.
- Achieve productivity goals as assigned.
- Maintain active Ohio LSW/LISW or LPC/LPCC license, in good standing with the state board.
- Perform duties in accordance with applicable professional ethics and standards as well as established policies.
- Comply with all state and federal laws regarding protected health information.
ADDITIONAL DUTIES AND RESPONSIBILITIES - Perform job duties in a manner that's friendly, considerate, and aligned with the agency's Mission, Vision, and Standards of Ethical Conduct.
- Organize and plan work for maximum efficiency and effectiveness.
- Utilize IT system and software programs in accordance with agency policy.
- Utilize time off in a manner consistent with agency policy.
- Perform other duties as deemed necessary and appropriate by supervisor.
ESSENTIAL KNOWLEDGE, SKILLS, ABILITIES, AND PERSONAL CHARACTERISTICS - Ability to learn and utilize the agency's Electronic Health Record database and other electronic tools effectively and efficiently within 90 days of hire.
