Essential Responsibilities:
  • Assist in the review, documentation, and investigation of all allegations of abuse and neglect, reportable incidents, notable occurrences, Part 625 events and occurrences, and non- reportable incidents.
  • Assist with the filing of allegations of abuse and neglect, reportable incidents, notable occurrences, and Part 625 events and occurrences using the Incident Report and Management Application.
  • Complete investigations and Investigative Reports as required.
  • Develop and oversee completion of recommended actions and corrective action plans.
  • Support the Director of Incident Management during non- business hours with Incident Management responsibilities as needed.
  • Maintain current trainings relative to the performance of incident management responsibilities and as required.
  • Develop an understanding of the Part 624/625 Handbook, commentary, and FAQs and provide support as appropriate.
  • Complete all other tasks relative to incident management as assigned.
  • Staff the on-call phone, on a rotational basis, during off-business hours.

Experience and Licenses:

Must have a minimum of 5 years' experience working in the ID/DD field.

Two years of experience in an Incident Management role, working for an OPWDD certified provider agency or CCO required.

Certification in Conducting Serious Incident Investigations (Level 1) preferred. Tri-County Care
